Note: Please select the necessary form(s), print, fill out and return to the Church Office or give to the appropriate ministry leader.

General Forms

1.       Background Check – A completed Background Check MUST be on file in the Church Office if you are over the age of 18 and work or volunteer with our children and/or youth and must be updated once every three years

2.       Request for Facilities Use – This form should be completed when requesting to use areas within the church (FLC, CLC, Sanctuary and/or Classrooms) for functions within a ministry, personal use or outside organization use. Please complete all necessary information and email to the Office Administrator or turn in to the Church Office.
